SKILLED NURSING SERVICES

  • Making the initial comprehensive assessment visit to the patient.
  • Provide the services that require more specialized nursing care, which include and not limited to:
    • IV Therapy
    • Tube Feeding/Tracheostomy Care, Colostomy care
    • Wound Care
    • Respiratory Equipment Management and Teaching
    • Disease Process Management
    • Medication Management including administration of IV, IM and SC Medications
  • Reevaluating their needs regularly
  • Initiating and revising the nursing plan of care
  • Planning for preventative and rehabilitative care
  • Preparing clinical notes and informing physicians and others participating in the patients care of changing as they occur

MEDICAL SOCIAL SERVICES

  • Assess patients/families ability to cope up with social, psychological, cultural and medical issues resulting from an illness
  • Providing counselling
  • Providing community resources as needed

PHYSICAL THERAPY SERVICES

  • Providing evaluation and treatment of patients rehabilitation needs
  • Therapeutic exercises for muscle conditioning and strengthening to regain functional mobility and gross motor skill
  • Home safety assessment and instruction

OCCUPATIONAL THERAPY SERVICES

  • Providing training for activity of daily living to maximize the patient’s functional independence within his or her home
  • Evaluation and training in adaptive equipment
  • Studies, evaluates and records patient’s activities and progress

SPEECH THERAPY SERVICES

  • Maximizing a patient’s functional independence in communicating and swallowing
  • Increasing the patient’s and caregiver’s knowledge of speech, language and swallowing disorders and of appropriate treatment processes

PHARMACEUTICALS & LABORATORY SERVICES

Coordinate delivery of Prescription Drugs or Laboratory services ordered by doctors done in the patient’s home
